Modalidades para la rehabilitación del piso pélvico como estrategia de mejora de efectos secundarios en pacientes candidatos a prostatectomía radical: revisión sistemática

Resumen

Introducción: el cáncer de próstata es el segundo tipo de cáncer más frecuentemente diagnosticado en hombres mundialmente, con importante incidencia en Colombia; la estrategia terapéutica principal continúa siendo la prostatectomía radical. Sin embargo, es común presentar efectos secundarios que afectan considerablemente la calidad de vida de las personas; por lo tanto, se han propuesto terapias como el entrenamiento de los músculos del suelo pélvico con o sin biorretroalimentación, estimulación eléctrica, inervación magnética y terapia de choque. Objetivo: evaluar y difundir las distintas modalidades disponibles hoy en día que posiblemente pueden ser
implementadas en la atención primaria. Métodos: se realizó la búsqueda de información en distintas bases de datos como PubMed, Wiley, Springer, Sagepub, Nature, Embase y Web of Science en relación con estudios publicados en los últimos 5 años. Se incluyeron ensayos aleatorizados en los cuales se describieran las distintas modalidades utilizadas con efectos antes y después de la realización del procedimiento de prostatectomía independiente de la técnica utilizada. Resultados: finalmente, se seleccionaron once artículos, en los cuales se utilizaron diferentes cuestionarios de evaluación para objetivar la afectación de los efectos secundarios posteriores a prostatectomía radical, así como, los beneficios tanto pre y posoperatorios de terapias combinadas. Discusión y conclusiones: las diferentes modalidades de rehabilitación, en especial aquellas que son combinadas, desempeñan
un papel relevante en la recuperación de efectos secundarios, no solo referentes a la continencia urinaria, sino también a la función sexual, impactando significativamente la calidad de vida de los pacientes. Sin embargo, es de anotar que la gran mayoría de información disponible está basada en características de otros países, sin tener en cuenta poblaciones como la latinoamericana.
