Osteomielitis neonatal: un verdadero desafío diagnóstico
Resumen
La osteomielitis neonatal (omn) es una entidad infecciosa poco frecuente que afecta el hueso durante el primer mes de vida. Su diagnóstico representa un reto clínico debido a la presentación inespecífica y la ausencia de signos sistémicos evidentes ,como la fiebre, lo que exige un alto nivel de sospecha y pericia diagnóstica. La principal vía de infección es la diseminación hematógena, favorecida por la anatomía vascular de la metáfisis de los huesos largos y la inmadurez inmunológica del neonato. Factores como la prematurez, el uso de dispositivos invasivos y antecedentes maternos infecciosos o autoinmunes aumentan el riesgo de esta patología. El diagnóstico suele ser tardío y requiere correlación entre hallazgos clínicos, paraclínicos e imagenológicos. El manejo oportuno y dirigido, como se ilustra en el caso presentado, puede evitar secuelas funcionales y óseas a largo plazo, subrayando la importancia de considerar patologías poco frecuentes en el contexto neonatal.
